If your SA Ambulance Account (Tax Invoice) charges will be settled by private health insurance, please let us know.

After you submit the form:
- Your Ambulance Account will be placed on hold temporarily, for your private health provider to settle the fees and charges.
- Please contact your private health provider to notify them of your Ambulance Account charges. You may be required to provide them with a copy of your SA Ambulance Tax Invoice.
- Our Customer Service Centre will contact you if any more information is required.
