After you submit the form:

  • Your Ambulance Account will be placed on hold temporarily, for your private health provider to settle the fees and charges.
  • Please contact your private health provider to notify them of your Ambulance Account charges. You may be required to provide them with a copy of your SA Ambulance Tax Invoice.
  • Our Customer Service Centre will contact you if any more information is required.
